Role Summary:PharmD Live seeks a Spanish-speaking LPN/LVN to provide remote virtual care for patients in CCM and RPM programs. The role focuses on bilingual patient education, care coordination, and clinical support.
Key Responsibilities:
Conduct telehealth visits in Spanish and English
Review patient-reported data and RPM alerts
Document care in compliance with CMS and internal standards
Collaborate with interdisciplinary care teams
Support post-discharge and follow-up coordination
Qualifications:
Active Nevada LPN or LVN license
Professional fluency in Spanish and English
Minimum 2 years clinical or telehealth experience
Strong documentation, clinical judgment, and communication skills
Experience with CCM or RPM preferred
Ability to work independently in a HIPAA-compliant environment
