Registered Nurse (RN) | Hospital at Home
The Registered Nurse (RN) Hospital at Home provides acute-level nursing care to patients receiving hospital services in their homes. This role delivers comprehensive, evidence-based clinical care consistent with inpatient standards while promoting patient safety, comfort, and recovery in the home setting. The RN functions as part of an interdisciplinary care team and is responsible for assessment, clinical intervention, care coordination, and patient education within the Hospital at Home model. Provides patient care by reviewing physician prescriptions; communicating the nursing care plan and all pertinent information to other members of the health care team; coordinating with other healthcare providers; providing physical, emotional, psychological, and spiritual support to patients, families, and friends; conferring with physicians.
