Conduct evaluations of members' needs using clinical tools and data review.

Incorporate strategies to reduce risk factors and address complex health and social indicators.

Conduct assessments considering various sources of information for co-morbid and multiple diagnoses.

Use a holistic approach to assess the need for referrals to clinical resources and interdisciplinary team members.

Collaborate with supervisors and key stakeholders to overcome barriers in meeting healthcare goals.

Utilize case management processes in compliance with regulatory and company policies.
,
Employ motivational interviewing skills to engage members and assess their health needs.
Requirements
Registered Nurse with an unrestricted nursing license and multi-state/compact privileges.
,
3+ years of clinical/nursing practice experience.
,
1+ year(s) of medical case management experience, care coordination, or discharge planning.
,
3+ years of experience with personal computers, keyboarding, multi-systems navigation, and MS Office Suite applications.
,
COVID vaccination is required.
