About the position
Your Choice Health Care, LLC is seeking a dedicated Registered Nurse (RN) Case Manager to join our team, focusing on the Gallia, Meigs, and Jackson County areas. This position offers a unique opportunity to provide care and treatment in patients' homes, allowing for a flexible work schedule that can accommodate both full-time and part-time employees. The RN will be responsible for a variety of tasks including wound care, IV therapy, disease teaching and management, telehealth services, medication education and management, and venipuncture skills. Additionally, the role involves foley catheter care, conducting assessments, and participating in the care planning process. The RN will also be responsible for electronic documentation and acting as a liaison between patients, physicians, and external case managers. Supervisory visits for paraprofessional staff will also be part of the responsibilities. The position requires a valid RN license to practice in the state of Ohio, a current driver's license, and the ability to pass a BCI background check and drug screen. Good written and verbal communication skills are essential for this role.
Responsibilities
