Registered Nurse | Case Manager | Remote | Michigan | Contract Walker Healthforce is seeking a RN Case Manager for a healthcare client located remotely. This is a contract opportunity. Start Date: ASAP CORE REQUIREMENTS • 3+ years of clinical nursing experience in a clinical, acute/post-acute care, and community setting • Michigan RN License • Nursing Diploma or Associate's Degree in Nursing • 1+ years case management experience in a managed care setting • Proficient in Microsoft programs (Excel, OneNote, Outlook, Teams, Word, etc.) WE CONSIDER IT A BONUS IF YOU ALSO HAVE: • Case Management Certification (CCM) • Bachelor's Degree Nursing • Certification in Chronic Care Professional (CCP) JOB DESCRIPTION: • Lead a multidisciplinary team in delivering holistic, person-centric care management • Act as the primary contact for members, caregivers, and providers • Utilize the case management process to assess, develop, implement, monitor, and evaluate care plans across the care continuum. • Assess members' health, psychosocial needs, and support systems, developing individualized care plans. • Arrange necessary resources and coordinate care delivery among support systems, providers, and community agencies. • Advocate for members and provide education on health literacy, self-management, medication plans, and nutrition. • Monitor and evaluate care plan effectiveness, ensuring adherence to goals. • Document interactions accurately to support member management. • Prepare members for discharge or transfer, ensuring continuity of care.
