JOB DESCRIPTION Job Title: Remote RN Case Manager (Spanish) Location: 100% Remote (Open to all U.S. time zones; preference for 1–2 candidates in CST) Pay Rate: $48/hr (W2) Duration: 6 Months (Extension possible) Citizenship: Must be a U.S. Citizen Openings: 1 Position Position Summary The RN Case Manager ensures that members with complex medical and/or psychosocial needs receive high-quality, cost-effective healthcare. The role involves holistic assessment, care planning, coordination, monitoring, and evaluation to support optimal member health outcomes. The Case Manager serves as an advocate, using sound clinical judgment and collaboration with healthcare providers and community resources to facilitate continuity of care in compliance with all regulatory standards. Required Qualifications • Education: Bachelor’s Degree in Nursing or RN Certification. • Experience: Minimum 3 years in Acute Care or Managed Care settings, preferably with Medicaid or Medicare populations. • Strong clinical judgment and communication skills. Preferred Qualifications • Bilingual in English and Spanish. • Case Management Certification (CCM or equivalent). Additional Details • 100% Remote Work • Must complete Care Manager Assessment in Glider prior to submission. • 6-month contract, potential for extension. • U.S. Citizenship required.
