Job Summary:
Integrate healthcare services for patients from pre-admission to post-discharge, ensuring coordinated and sequenced care.
Evaluate patient outcomes based on clinical, financial, and utilization data, utilizing independent nursing judgment and age-specific care methodologies.
Communicate effectively with patients, families, and healthcare teams, facilitating groups and implementing projects as needed.
Demonstrate fiscal responsibility, controlling variances and duplication, and documenting clearly and concisely.
Work autonomously, prioritizing tasks, and adapting to changing situations, with strong flexibility and organizational skills.
Requirements:
Bachelor's degree in nursing preferred, with a minimum of 1-2 years of professional nursing experience in a clinic or home health setting.
Current RN license with the State Board of Nursing, with multistate licensure preferred if in a Nurse Licensure Compact (NLC) state.
Department-specific competencies and certifications, with a valid driver's license required depending on location.
