As we continue to expand our clinical programs to meet increasing community needs, we are seeking RN Case Managers to join our teams in Onondaga, Oneida, Oswego, Jefferson, or Cayuga Counties! Home visits are scheduled Monday�Friday, 8:00 AM�4:30 PM, with no on-call requirement, and a one weekend a month rotation. What You�ll Do as an Adult RN Case Manager Provide high-quality nursing care to adult patients through a combination of hands-on clinical care, assessment, education, and care coordination. You�ll be trusted, valued, and empowered to use your clinical judgment to make a meaningful difference. Your responsibilities include: Deliver safe, patient-centered nursing care in the home Serve as the primary case manager, coordinating care with the interdisciplinary team Develop and implement individualized plans of care Provide patient and caregiver education, support, and disease management teaching Ensure accurate, timely documentation (documentation time built into the home visit) Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient�s plan of care Maintain compliance with all infection control and regulatory standards Stay current with clinical best practices What We�re Looking For! Current NYS Registered Nurse license (from an accredited program) BSN preferred (tuition reimbursement available!) Valid driver�s license, auto insurance, and reliable vehicle Self-directed, organized, and comfortable working independently Adult medical-surgical, home care, case management, or chronic disease management experience a plus Strong communication and computer skills Physical Requirements: Ability to sit or stand for long periods Ability to lift/carry 25�40 lbs and assist with transfers up to 150 lbs Comfortable with bending, kneeling, climbing stairs, and driving in all weather Ability to use a phone, laptop, tablet, and electronic medical record Category III bloodborne pathogen exposure (rare contact with blood/body fluids) Compensation & Benefits: Competitive Salary [This position is an S03 exempt position with a min-max rate of: $75,250- $87,138.92/annually] with: $15,000 sign-on bonus* Retention Bonuses Internal Mentoring Program 401K with generous Employer match On-Site Gym (free for all employees) Excellent work/life balance (no rotating shifts, only 2 required holidays per year on a rotating schedule) Medical, Dental, Vision plans Tuition Reimbursement (BSN after only 6 months of employment!)
