For more information, visit www.jobs.premisehealth.com.As a Full Time Registered Nurse, Care Manager and Wellness Coach (RN), you'll provide care to client employees in our Health Center located in Colorado Springs, CO. The scheduled hours anticipated will be Monday- Friday 7:30am-4:30pm.Essential FunctionsDevelops personalized treatment plans using provider care plans and member preferences using a coach approach and the pillars of Lifestyle MedicineSupports a proactive, multidisciplinary team approach directed toward prevention, education, and health promotionProvides condition specific self-care education to members enrolled in the various condition management programsParticipates in outreach/follow-up phone calls and follow-up member visitsDocuments touchpoints and progress into the electronic medical recordCollaborates with the full scope of clinical team members to arrange for further medical treatment when neededStrives to work with member to improve health outcomes and better management of any chronic conditionDelivers one on one and/or group coaching sessions in the areas of weight, nutrition, exercise, stress, tobacco cessation, work/life balance and general healthWorks closely with client’s vendors as appropriate to support member’s needs where appropriateIdentifies emergency situations and provides care within legal scope of practiceActs as facilitator of Care Team meetings.Coach effectively in-person and by phoneMay coordinate and conduct wellness programs in areas such as nutrition, stress management, weight management, smoking cessation, exercise and othersJob RequirementsCurrently licensed Registered Nurse in state of practiceCurrent Wellness Coach Certification from Wellcoaches® OR equivalent International Coach Federation-accredited program certification (must be obtained within 24 months of hire)Current hands on training in AHA or ARC Basic Life Support for health care providers required; Advanced Cardiac Life Support may also be required based on contract scope of service.Bachelor's degree (BSN) from four-year college or university (preferred)At least three years’ experience as a Registered Nurse caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension.At least three years experience as a Registered Nurse in an emergency room, occupational health clinic or other ambulatory care clinic. Knowledge of Internet software, Spreadsheet software and Word Processing software. Knowledge and experience with Electronic Medical Records preferred. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.For individuals living in Washington or Colorado: Premise offers the following paid time off benefits. Employees will earn 0.0692 hours of paid time off per actual hours worked or based on standard schedule, per pay period, and will receive 10 paid holidays or an equivalent bank of hours aligned to schedule throughout the calendar year.
