Registered Nurse Assessor (Hybrid Remote) - Transform Lives with Maximus

Maximus is seeking a compassionate and experienced Registered Nurse Assessor to join our team in the Bronx, NY. As a hybrid remote role, you will have the flexibility to work from home and conduct in-home assessments throughout the assigned region. Key Responsibilities:
ā¢ Conduct comprehensive assessments to determine eligibility for community-based long-term services
ā¢ Render medical necessity determinations and resolve technical issues
ā¢ Perform initial assessments, evaluate health status, and document outcomes
ā¢ Maintain current knowledge of regulatory and project changes
ā¢ Collaborate with interdisciplinary care team members to facilitate receipt of services

Requirements:
ā¢ Active RN license in the state of New York
ā¢ Minimum 2 years of clinical experience with RN license
ā¢ Minimum 2 years of home setting experience, including work with CHHAs, LHCAs, MLTC, or as a paraprofessional
ā¢ Ability to conduct in-home health assessments and travel within assigned region
ā¢ Proficiency in navigating web-based applications and Microsoft Outlook & Word
Home Office Requirements:
ā¢ Reliable high-speed internet service
ā¢ Minimum 20 Mbps download speeds/50 Mbps for shared internet connectivity
ā¢ Minimum 5 Mbps upload speeds
ā¢ Candidates must have their own cell phone
