As a hybrid remote role, you will have the flexibility to work from home and conduct in-home assessments throughout the assigned region. Key Responsibilities:
Conduct comprehensive assessments to determine eligibility for community-based long-term services
Render medical necessity determinations and resolve technical issues
Perform initial assessments, evaluate health status, and document outcomes
Maintain current knowledge of regulatory and project changes
Collaborate with interdisciplinary care team members to facilitate receipt of services

Requirements:
Active RN license in the state of New York
Minimum 2 years of clinical experience with RN license
Minimum 2 years of home setting experience, including work with CHHAs, LHCAs, MLTC, or as a paraprofessional
Ability to conduct in-home health assessments and travel within assigned region
Proficiency in navigating web-based applications and Microsoft Outlook & Word
Home Office Requirements:
Reliable high-speed internet service
Minimum 20 Mbps download speeds/50 Mbps for shared internet connectivity
Minimum 5 Mbps upload speeds
Candidates must have their own cell phone
