Hospice Registered Nurse (RN) Shifts Available: Full Time Service Areas with Openings: Northwest & Southwest Houston Responsibilities: Oversee nursing care team. Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care. Plan, organize, and direct home care services. Conduct assessments of patient care needs and develop care plans. Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes. Prepare timely, complete, and accurate clinical notes. Night and Weekends are required per hospice needs of patients Qualifications: Valid RN license in the State of Texas. Hospice experience (at least 1 year). Knowledge of home health regulations and general healthcare compliance. Experience with Home Care Home Base (HCHB) EMR (preferred but not required). Excellent communication and people skills are essential. Ability to work independently and as part of a team.
