PT / Physical Therapist - Home Health - New Grads Welcome!
About the position
Adoration Home Health is seeking a passionate, dedicated Home Health PT to join our team in Westminster, MD. Our Home Health PTs provide expert, patient-centered care. New graduates are welcome to launch their PT career with support and growth opportunities. The office is located in Westminster, MD, with a coverage area including Frederick and Washington County. This is a Full-Time position, Monday - Friday. Home Health and HCHB Experience is preferred.
Responsibilities
ā¢ Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
ā¢ Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
ā¢ Maintain communication among the treatment team, physician, patient, and family members as applicable
ā¢ Maintain thorough and timely documentation of services provided to ensure proper billing
ā¢ Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act
Requirements
ā¢ Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
ā¢ At least (2) years of appropriate experience as a physical therapist.
ā¢ Valid license form the state of practice
ā¢ Possess and maintains current CPR certification
Nice-to-haves
ā¢ New physical therapy graduates may be considered in select markets based on program availability
ā¢ Community/hospice experience is preferred
ā¢ Home Health and HCHB Experience Preferred
