The Responsibilities:Conduct initial evaluations and collaboratively develop treatment plans with clients for medication, therapy or a combination of both. Proactively follow up with clients at minimum on a monthly basis to assess for efficacy of recommended interventions and ongoing care until treatment goals are achieved.
