PRN Remote Hospice Triage Nurse, RN

We’re hiring a Regional Triage Nurse (RN) – a skilled and compassionate professional who provides telephonic triage, clinical oversight, and coordination of after-hours care across multiple hospice sites. This role is essential to ensuring timely, high-quality care for patients and families during critical moments. Responsibilities • Oversee and assign on-call/after-hours RN runners for supported hospice sites • Provide telephonic assessments and implement appropriate interventions based on patient’s terminal diagnosis and reported symptoms • Collaborate with attending physicians, Medical Directors, and interdisciplinary hospice teams • Utilize EMR systems (HCHB workflow) as assigned • Act as liaison with patients, families, and healthcare professionals while maintaining confidentiality and dignity Requirements • Active RN licenses in good standing in both California and Texas • Minimum of 1 year RN experience, including at least 1 year in end-of-life care • Must clear criminal background check, physical, and drug screening • Valid driver’s license, good driving record, and reliable transportation required • Willingness to travel to patient locations as needed • Ability to work outside regular hours depending on business needs Nice-to-haves • Certification as a Hospice and Palliative Nurse (CHPNA) preferred but not required Benefits • 401(k) matching • Health, Dental and Vision Insurance • Flexible Spending Account (FSA) • Paid time off • Tuition reimbursement and professional development assistance • Referral program • Pet Insurance • Auto and Home Insurance and Discount Programs Apply To this Job Apply To this Job
