Population Health Manager

Develop and implement effective population health management strategies to improve the health and well-being of individuals and communities. Stay up-to-date on industry trends and research in population health management to inform decision-making. Collaborate with cross-functional teams to identify and address population health needs and gaps in care. Analyze data and metrics to measure the success and impact of population health initiatives. Lead and manage a team to ensure the successful implementation and execution of population health programs. Foster a culture of continuous improvement by identifying opportunities for process enhancement and innovation. Build and maintain relationships with key stakeholders, including healthcare providers, community organizations, and government agencies. Develop and deliver presentations and trainings to inform and educate internal and external stakeholders on population health management. Monitor and manage the budget for population health initiatives, ensuring resources are utilized efficiently and effectively. Stay compliant with all regulatory and legal requirements related to population health management. Continuously evaluate and adapt population health strategies to meet the evolving needs of our members and communities. Serve as a subject matter expert on population health management, providing guidance and support to colleagues and team members. Act as a role model for the company's values and promote a culture of inclusivity and diversity. Participate in strategic planning and goal setting for population health management initiatives.
