Classification Summary

Human Services of Faribault and Martin Counties is seeking applicants for a full-time (40 hours per week) Planner (Human Services) - Public Health Assessment & Surveillance Coordinator position. Entry salary range is $30.31 - $33.06 per hour. Join a forward-thinking, collaboration-driven public health team working to strengthen the wellbeing of communities across South Central Minnesota. As the Planner – Public Health Assessment & Surveillance Coordinator, you will lead regional population health assessment and surveillance efforts that directly shape local decision making, drive community health improvement strategies, and support the public health infrastructure of multiple partner jurisdictions. In this innovative regional role, you will build data capacity across local health departments, manage datasets from state and federal sources, and transform information into clear, actionable insights for leaders, boards of health, and community partners. You will also facilitate regional data meetings, create accessible data tools, support grant applications, and help identify emerging health trends that guide proactive public health action. This position offers the opportunity to supervise and mentor student interns, collaborate with university partners, and contribute to a strong academic–public health pipeline. Examples of Duties

MN Merit System Examples of Duties: Works with community and professional groups in developing specific planning philosophies, strategies and goals. Assists the agency leadership with preparation of public health or human services program plans, goals, objectives, reports and budget. Consults with state, regional, and local officials, staff members and organizational leaders to gain information related to specific plans and programs. Confers with local officials and agency staff on specific planning projects, techniques and goals. Assists in implementing and evaluating new programs and services related to planning efforts. Conducts surveys and/or community assessments as needed for program development. Monitors the goals and objectives of the public health program plans and evaluate progress towards meeting these with staff and resources available. Give community presentations on assigned program areas as requested.
Minimum Qualifications

MN Merit System Minimum Qualifications A Bachelor's degree from an accredited four-year college or university in social work, psychology, sociology, public health, health education, planning, public administration, community health, urban planning, policy analysis, quantitive or statistical analysis or closely related field and three (3) years of experience with professional responsibility for planning or administrative functions such as coordinating or planning community programming for specific populations. OR A Master's degree from an accredited four-year college or university in social work, psychology, sociology, public health, health education, planning, public administration, community health, urban planning, policy analysis, quantitive or statistical analysis or closely related health or human services field AND one (1) year of experience as a social worker, public health nurse, community health educator or related experience with some professional responsibility for planning or administrative functions such as coordinating or planning community programming for specific populations.
