Physical Therapist, RPT, Flexible Weekday Schedule
About the position
Concierge Home Care is hiring a Home Health Physical Therapist (PT) in the St Augustine area. This role offers more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity. The coverage area is St Augustine / St Johns County, with territories designed to minimize drive time through clustered scheduling. The position emphasizes flexibility and autonomy, with no weekends or holidays required (optional flexibility), no hospital shifts or 12-hour rotations, and greater control over work-life balance. Technology such as AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) is utilized to reduce documentation time. The company also provides structured onboarding, mentorship, preceptorship with experienced clinicians, ongoing training and development, free CEUs, and career advancement opportunities.
Responsibilities
• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
Requirements
• Active, unencunbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
• Successful completion of an AHCA Level 2 background screening
