Position: Patient Community Navigator, Remote Patient Monitoring Program - Native Hawaiian Health (Tempor[...] Responsibilities JOB SUMMARY/RESPONSIBILITIES: Supports community health by establishing and maintaining trusting relationships with patients, families, and providers to promote health, recovery, resiliency, and wellness. Performs activities off-site, in the community, and makes patient home visits as required. Performs duties and responsibilities in accordance with policies and procedures and consistent with the goals and objective of the department and the Queen’s University Medical Group (QUMG). Typical Physical Demands β€’ Essential: stooping/bending, finger dexterity, seeing, hearing, speaking, lifting usual weight of 10 pounds up to 25 pounds. β€’ Frequent: standing, walking, reaching at and below shoulder level. β€’ Occasional: sitting, kneeling, climbing stairs, walking on uneven ground, squatting, pushing/pulling usual weight of 1 pounds up to 2.5 pounds of force, carrying usual weight of 5 pounds, reaching above shoulder level. Typical Working Conditions β€’ Not substantially subjected to adverse environmental conditions. β€’ Work involves extensive off-site activities, including home and community-based visits with patients. β€’ Must have reliable transportation in order to make visits to sites where patients reside. β€’ May be exposed to negative responses from patient or family. Minimum Qualifications Education/Certification and Licensure β€’ High School diploma or equivalent. β€’ Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations. β€’ Driver's abstract or evidence of good driving record. β€’ Medical Assistant, Community Health Worker or Patient Navigation certification preferred. β€’ Current BLS certification. Experience β€’ Two (2) years of relevant healthcare/social services experience; or one (1) year of healthcare/social services experience with a Community Health Worker certificate. β€’ Previous experience working with individuals with behavioral health and/or substance use issues preferred. β€’ Prior experience working with Native Hawaiian and other underserved populations preferred.
