Coordinate and manage the care of patients within the Patient-Centered Medical Home (PCMH) model.
Collaborate with interdisciplinary team members to develop and implement comprehensive care plans for patients.
Monitor and track patient progress, ensuring timely and appropriate follow-up care.
Serve as the primary point of contact for patients and their families, addressing any questions or concerns they may have.
Educate patients on their care plans, including medication management, appointment scheduling, and other necessary resources.
Advocate for patients and advocate for their needs within the healthcare system.
Conduct comprehensive assessments of patient needs, including physical, emotional, and social factors.
Ensure accurate and timely documentation of patient interactions and care plans.
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Stay up-to-date on industry standards and best practices in patient-centered care.
Collaborate with community resources and healthcare providers to ensure seamless transitions of care for patients.
Provide support and guidance to other team members, including CMAs and medical assistants.
Adhere to all HIPAA and patient privacy regulations.
