Are you a detail-oriented Licensed Vocational Nurse who thrives in a fast-paced, collaborative environment? We are looking for a dedicated professional to join our hospice team as a part-time Weekend Clinical Support LVN . In this role, you will be the heartbeat of our branch operations, ensuring our patients receive seamless, high-quality care through expert scheduling, coordination, and clinical administrative support.

Established in 1934, the Visiting Nurse Association of Texas (VNA) is a non-profit organization providing compassionate care to the community. As the first and most experienced Hospice Care provider in Texas, VNA has a trusted reputation for delivering high-quality care for over 90 years. Our hospice and palliative care programs are available across 16 counties in North Texas, including: Collin, Cooke, Dallas, Denton, Ellis, Fannin, Grayson, Henderson, Hunt, Johnson, Kaufman, Rockwall, Tarrant, Van Zandt, and parts of Wise and Navarro Counties.

What You'll Do Coordinate Care: Schedule field visits, prioritize nursing assignments, and ensure all referrals and admits are processed efficiently. Operational Excellence: Manage the weekend log, run compliance reports, and facilitate DME (Durable Medical Equipment) orders to ensure patients have what they need, when they need it. Clinical Collaboration: Work closely with RNs, Physicians, and the IDT to update patient findings and manage documentation such as DNRs and triplicates. Patient Advocacy: Act as a key point of contact for families, providing information, education, and addressing concerns with empathy and professionalism. Why Work at VNA Texas? Qualifications, Skills, and Abilities: Education: Graduate of an accredited school of vocational nursing. Licensure: Current State license as an LVN and a valid State Driver's License. Experience: Minimum of two years of experience as an LVN in a clinical setting; hospice or home health experience is strongly preferred. Skills: Strong computer proficiency, excellent interpersonal communication, and top-tier organizational abilities. Requirements: Reliable transportation and current auto liability insurance; must be able to pass a criminal background check. Work Environment This position involves a hybrid schedule, with duties performed both remotely and in the office. The role requires long periods of sitting while conducting computer work. You will spend a significant amount of time on the phone and in front of computer screens to manage branch communications and data. Physical and Mental Effort Mental Resilience: You must be able to work under stressful conditions to meet tight deadlines and urgent patient needs. Quick Decision-Making: This role requires the ability to make rapid decisions and efficiently acquire necessary resources. Empathy: You will be responsible for meeting the individualized psychosocial needs of both patients and their families. Dexterity: Significant computer time is required, necessitating strong manual dexterity for data entry and navigation.
