With over 4,500 residents under our care, we provide on-site, consistent medical services to improve outcomes and reduce hospitalizations. Position Summary: We’re hiring a Physician Assistant (PA-C) who enjoys being on the move and working in non-traditional clinical settings. You'll serve residents in assisted living and memory care communities, building relationships and delivering meaningful, autonomy-driven care in place. Responsibilities: Travel to multiple senior living communities weekly to provide medical care Conduct assessments, manage chronic conditions, and address acute concerns Partner with interdisciplinary teams to align on patient goals Maintain accurate, timely EMR documentation and communicate care plans Qualifications: Active MN PA license (Board of Medical Practice) Background in geriatrics, internal medicine, or complex care preferred Strong communication and time management skills Must have reliable transportation and current auto insurance Military Occupational Classification (MOC) Codes: We proudly welcome veterans and transitioning service members.
