Occupational Therapist – Remote Assessment & Intervention
About the position
Liquid Personnel is seeking an experienced Occupational Therapist to join their remote assessment service. You will triage new referrals, complete telephone/video assessments, prescribe equipment and minor adaptations, and contribute to prevention‑focused work across Adult Social Care. This is a fast‑paced role requiring strong clinical reasoning, risk management, and digital communication skills.
Responsibilities
• Review referral information to identify priority, risk, and required OT intervention.
• Carry out remote assessments via telephone and video call.
• Provide preventative advice, guidance, and problem‑solving support to adults, carers, and professionals.
• Escalate safeguarding concerns in line with statutory procedures.
• Apply strong functional questioning and clinical reasoning to assess needs remotely.
• Prescribe standard equipment and minor adaptations in accordance with service guidelines.
• Make defensible, evidence‑based clinical decisions that reflect risk, strengths, and need.
• Maintain accurate and timely case records.
• Contribute to prevention-focused work, including equipment reviews, moving & handling advice, and self‑management strategies.
• Work collaboratively with community health, housing, social work, reablement, voluntary sector teams, and safeguarding partners.
• Participate in supervision, MDT meetings, training, and service‑development activities.
• Promote a solution‑focused and strengths‑based culture.
Requirements
• Experience working in Adult Social Care, community OT, hospital discharge, reablement, or similar settings (Essential).
• Experience managing complex cases and risk in fast‑paced environments (Essential).
• Strong understanding of: Care Act 2014, Housing legislation, Equipment provision & adaptation pathways, Strength‑based approaches
• Excellent digital and telephone communication skills.
• Ability to assess risk, problem‑solve creatively, and make safe, defensible decisions.
• Strong organisational skills and ability to manage competing priorities.
• Effective independent working and strong team-working skills.
• Resilience, flexibility, and the ability to work under pressure.
• Strong moving & handling knowledge
