Occupational Therapist (OTR/L) – In-Home Rehabilitation Specialist

This is a field-based opportunity for an Occupational Therapist (OTR/L) who values autonomy, efficiency, and meaningful one-on-one care. Unlike clinic-based roles, this position offers more control over your day, stronger patient relationships, and the ability to significantly impact patient outcomes. At Home Health Care, Inc., you will own your schedule, manage your territory, and deliver care directly within patients' homes. This role is ideal for top performers seeking a less volume-driven environment and more independence.
Responsibilities
• Conduct in-home OT evaluations focused on safety, function, and independence.
• Develop treatment plans that drive measurable ADL and mobility outcomes.
• Provide hands-on therapy to improve strength, coordination, and daily function.
• Perform home safety assessments and recommend adaptive equipment.
• Manage territory efficiently to maximize time and impact.
• Complete accurate EMR documentation and collaborate with the care team.
Requirements
• Active Minnesota OTR/L license.
• Strong self-management and ability to work independently.
• Organized, efficient, and outcome-driven mindset.
• Reliable transportation for field-based visits.
• Passion for helping patients regain independence in their home.
