About the position
PIVOT Rehabilitation is hiring a Licensed Home Health Occupational Therapist (OT) for a PRN home health occupational therapist job providing in-home occupational therapy services to adult and senior patients in Houston, Katy, Fulshear, Cinco Ranch, and Brookshire Tx. This home health OT job is ideal for clinicians seeking flexible occupational therapist jobs in Houston, autonomy, and one-on-one patient care in the home setting.
Responsibilities
• Perform home health occupational therapy evaluations
• Develop and update individualized Plans of Care (POC)
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility
• Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments
• Implement interventions for fall prevention, home safety, and environmental modifications
• Train patients and caregivers in adaptive equipment and durable medical equipment (DME)
• Fabricate splints and recommend assistive devices as appropriate
• Complete accurate and timely documentation in the EMR
• Communicate with physicians and interdisciplinary care teams
• Supervise COTAs and home health aides, as applicable
• Follow all home health regulations, infection control, and safety standards
Requirements
• Bachelor’s or Master’s Degree in Occupational Therapy from an accredited program
• Active Texas Occupational Therapist (OT) license
• Current CPR certification
• Strong communication, clinical judgment, and time-management skills
• Valid driver’s license, reliable transportation, and current auto insurance
Nice-to-haves
• Home health occupational therapy experience preferred (1+ year required)
• Knowledge of home health regulations and Medicare guidelines preferred
