Responsibilities:
Conduct virtual patient assessments and develop personalized care plans.
Diagnose and treat acute and chronic health conditions via telehealth platforms.
Educate patients on health management and preventive care strategies.
Collaborate with multidisciplinary teams to ensure comprehensive patient care.
Document patient interactions and care plans in electronic health records (EHR).
Stay updated on telehealth regulations and best practices to ensure compliance.
Provide follow-up care and support to patients as needed.
Participate in quality improvement initiatives to enhance patient outcomes.
Requirements:
Master's degree in Nursing with a focus on Nurse Practitioner training.
Current Nurse Practitioner license in Georgia and board certification.
Minimum of 2 years of clinical experience, preferably in telehealth.
Strong communication skills and a compassionate demeanor.
Proficiency in using telehealth technology and electronic health records.
Ability to work independently and manage time effectively in a remote setting.
Commitment to providing patient-centered care and continuous learning.
Knowledge of telehealth regulations and best practices is a plus.
