Nurse Navigator - Hybrid (IL or OH)
About the position The Nurse Navigator plays a key role in delivering exceptional patient care by coordinating services for individuals with complex and chronic conditions. Through care management and education, this position ensures patients adhere to prescribed therapies, improving health outcomes and quality of life. Responsibilities • Conduct assessments and create care plans tailored to patient needs. • Serve as a resource and advocate, connecting patients to necessary healthcare services. • Continuously evaluate and refine care plans to enhance effectiveness. • Maintain accurate records and timely communication with stakeholders. Requirements • Accredited Diploma in Nursing required • 2+ years in an acute care setting required • Proficiency with Microsoft Office or similar software. • Critical thinking, multitasking, and effective communication skills. • Experience with electronic health records (EHR). • Ability to manage, review, and respond to emails/instant messages in a timely fashion.
