About the Job:
ā¢ Patient Care Coordinator - Health Coach.
ā¢ We are seeking a Patient Care Coordinator - LVN/LPN or RN.
ā¢ Must have a current nursing license.
ā¢ You will coordinate a care and coaching program with remotely monitored patients.
ā¢ This program focuses on diet and exercise. An increased knowledge based of macronutrients, calories, strength training, cardio, etc. is preferred.
ā¢ This is a 100% work from home position. Anywhere in the USA.
ā¢ Some latitude to create your own schedule.
Responsibilities:
ā¢ Communicate care information from programs.
ā¢ Engage new patients enrolled.
ā¢ Help patients track and report changes in condition.
ā¢ Update medical record systems with condition changes.
ā¢ Assist patients with healthcare questions and concerns.
ā¢ Help patients achieve goals.
Qualifications:
ā¢ Previous experience in healthcare.
ā¢ Experience in health coaching and weight management is an asset.
ā¢ Experience in a patient-facing role.
ā¢ Previous medical clinic and work from home experience is an asset.
ā¢ Current Nursing license.
ā¢ Strong interpersonal qualities.
ā¢ Strong organizational and time management ability.
ā¢ Strong technical skills.
ā¢ Excellent written and verbal communication skills.
ā¢ Understanding of medical record systems.
