Nurse Case Manager

At BlueCross BlueShield of South Carolina, we are looking for a highly experienced Nurse Case Manager to join our team. As a Nurse Case Manager, you will be responsible for providing comprehensive medical case management services to members to ensure they have access to appropriate and cost-effective care. You will be a critical part of the team, working closely with members and their families to ensure they receive the care they need.The ideal candidate for this position will be an experienced Registered Nurse (RN) with a minimum of five years of experience in case management and the utilization of healthcare resources. You should possess excellent research, analytical, and problem-solving abilities, as well as excellent written and oral communication skills. You should be organized, detail-oriented, and able to work well both independently and as part of a team.We are looking for a Nurse Case Manager who is passionate about providing quality care to members and has a commitment to providing excellent customer service. Responsibilities:
Provide comprehensive medical case management services to members in order to ensure they have access to appropriate and cost-effective care.
Research, analyze, and solve problems in order to provide members with the best care possible.
Develop and maintain relationships with members and their families in order to ensure they receive the care they need.
Utilize healthcare resources in order to maximize the effectiveness of care.
Provide excellent customer service to members in order to ensure satisfaction.
Collaborate with other healthcare professionals in order to provide quality care.
Maintain up-to-date knowledge of medical case management and healthcare resources.
Remain organized, detail-oriented, and able to work both independently and as part of a team.
Document and track progress of members in order to measure the effectiveness of care.
