Nurse Case Management Lead Analyst
Job Description:
• Lead strategic initiatives to improve patient care coordination and optimize resource utilization
• Analyze complex clinical data to identify trends and recommend actionable solutions
• Collaborate with cross-functional teams to design and implement innovative case management programs
• Drive continuous improvement by monitoring outcomes and adjusting strategies
• Serve as a trusted advisor, providing guidance and mentorship to team members
Requirements:
• Registered Nurse (RN) license in good standing
• Minimum 5 years of experience in case management or related clinical role
• Proven ability to analyze data and develop strategic care plans
• Strong communication and collaboration skills
