Position: Telehealth Nurse Practitioner Address: Texas Fully Remote Schedule: 24 hrs per week. (possibility of option for 36 hrs per week) β’ either 6a-6p or 9a-9p every other weekend, and 6p-12a or 6p-6a on weeknights as needed. Compensation: $66.67 Duration: 1099 Contract (1560 hours approx 9 months) Responsibilities: Position Summary We are seeking a Nurse Practitioner to join a national after-hours care team supporting residents in senior living and skilled nursing settings. This is a fully remote position responsible for delivering telephonic and virtual care during evenings, nights, weekends, and holidays. In this role, you will serve as a key clinical resource, providing timely medical guidance and support to both residents and facility staff. You will play a critical role in managing acute concerns, reducing unnecessary hospital transfers, and ensuring continuity of care outside of standard business hours. This position requires comfort with high call volumes, independent decision-making, and conducting telehealth evaluations, including assessment of acute changes in condition, falls, and medication-related needs. Key Responsibilities β’ Provide after-hours clinical support to patients and facility staff via phone and telehealth platforms β’ Assess and manage acute, chronic, and behavioral health concerns in collaboration with physicians and care teams β’ Coordinate care across care settings, including communication with facilities, hospitals, and outpatient providers β’ Conduct thorough clinical assessments and document all encounters in the electronic medical record in compliance with regulatory standards β’ Apply established clinical protocols and guidelines to support safe, effective treatment in place when appropriate β’ Participate in ongoing training and education to maintain clinical competency and compliance Schedule & Availability β’ Approximately 24 hours per week with a consistent, set schedule β’ Evening and overnight weekday shifts β’ Rotating weekend coverage with extended shifts β’ Participation in holiday coverage as part of team expectations Qualifications Required: β’ Masterβs degree as a Nurse Practitioner β’ Active, unrestricted Nurse Practitioner license in at least two states, with willingness to obtain additional licenses as needed- (IA MO KS or OH) or (NC and SC) or (NY and PA) β’ National certification (e.g., ANP, FNP, GNP) β’ Eligibility to obtain prescriptive authority and DEA licensure β’ Minimum of 3 years of clinical experience as a Nurse Practitioner β’ Experience managing acute and chronic conditions in adult or geriatric populations β’ Ability to work independently and manage multiple patient interactions simultaneously β’ Proficiency with electronic medical records and clinical documentation Preferred: β’ Experience working in skilled nursing facilities, senior living, or post-acute care settings β’ Strong understanding of geriatric care, chronic disease management, and acute condition evaluation β’ Experience with advanced illness care and end-of-life discussions β’ Strong communication skills and ability to build relationships with interdisciplinary teams β’ Adaptability in a fast-paced, evolving care environment
