40992 Job Description Job Summary: Clinical Diabetes Educator will provide telephonic education and support to members with chronic illness and will be responsible for improving health outcomes of and promoting appropriate preventive care for Medicare's members with Diabetes This role will also include communication with health care professionals to ensure goals of a member's treatment plan are met, as well as ensuring that members are linked with the appropriate resources and are adopting healthy lifestyle practices to reduce risk factors. Essential Functions: As a diabetes educator, your primary goal is to empower individuals with diabetes to manage their condition effectively. Key responsibilities include: Assessment and Education Planning: Develop personalized plans to help members understand their illness and set goals while complying with policies and procedures to meet HEDIS and NCQA standards Coaching and Support: Provide ongoing coaching, guidance, and support for medication adherence, symptom management, and lifestyle changes. Nutrition and Goal Setting: Counsel on daily nutrition, medical nutrition therapy, and help set achievable goals. Community Resource Navigation: Guide members in accessing community-based services. Empowerment and Education: Focus on prevention, health promotion, and education to empower members to take control of their health. By fulfilling these responsibilities, you will help individuals with diabetes manage their condition and improve their quality of life. Required experience: 3+ years related work experience and a Related Bachelor's degree or equivalent experience. Experience Details: Working with chronic populations to provide health education, self management, wellness, care coordination support and/or lifestyle related topics. Required Licenses and Certifications: Active NP, RPh, RN or RD, Active Diabetic Educator CDCES license General Physical Demands: Sedentary work: Exerting up to 10 pounds of force occasionally to move objects. Jobs are sedentary if traversing activities are required only occasionally. Typical Annualized Hiring Range: $63,800 - $79,700 Annualized Salary Range: $63,800 - $103,600 Final pay will be determined with consideration of market competitiveness, internal equity, and the job-related knowledge, skills, training, and experience you bring.
