Job Summary We are seeking a highly skilled and compassionate Medical Social Worker (MSW) to join our team in the Greater Oakland County Area, Michigan. As a Home Health and Hospice MSW, you will provide home-based social services to patients dealing with depression, anxiety, poor nutrition, or a lack of resources that make managing their injury or disease difficult. You will work as part of a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, PTs, OTs, and SLPs, to develop and implement patient care plans that address their unique needs and goals.
