CareCo Medical Home Health is seeking a licensed Medical Social Worker (MSW) to provide in-home social work services to patients in the community. This per diem role is an excellent opportunity for a social worker who enjoys working independently while collaborating with a multidisciplinary care team.

Position Summary
The Medical Social Worker addresses the social, emotional, and environmental factors that impact a patients ability to achieve the goals of the Plan of Care. Services are provided under physician direction and in collaboration with nurses, therapists, and other care team members, following agency policies and professional standards.
Pay and Schedule

Key Responsibilities
Assess the psychosocial status of patients and identify barriers to careComplete assessments and documentation in a timely mannerDevelop and implement care plans in collaboration with the interdisciplinary teamProvide counseling, guidance, and support to patients and familiesEducate patients, families, and team members on available community resourcesInitiate referrals and assist with discharge planningParticipate in care conferences, team meetings, and quality improvement activitiesCommunicate changes in patient status to physicians and care team membersMaintain compliance with agency, state, and federal standards
Qualifications
Masters Degree in Social Work from a program accredited by the Council on Social Work EducationMinimum of three years of experience in a healthcare setting or equivalent experienceHome health experience preferredStrong written, verbal, and interpersonal communication skillsKnowledge of community resources and social service systemsValid drivers license, reliable transportation, and current auto insurance
