Medical Care Coordinator (Remote)
We’re seeking a Medical Care Coordinator to support members as they navigate their healthcare needs, benefits, and available resources. This role is ideal for someone who is patient‑focused, organized, and passionate about improving access to care. What You’ll Do • Serve as the first point of contact for members in care management • Help members understand benefits, coverage, and care options • Conduct proactive outreach to address gaps in care and prior authorization needs • Provide disease‑specific education (adult & pediatric) under a licensed Care Manager • Identify social determinants of health and help remove barriers • Manage a caseload of 150–300 members with stable but complex or rising‑risk conditions What You Bring • Background in Social Work or Nursing preferred • Associate’s degree in a healthcare field or LPN required • Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m. If you’re passionate about guiding members through their healthcare journey and supporting better outcomes, this role offers a meaningful opportunity to make an impact.
