Prior CCM/RPM Experience (Preferred)

As a Care Manager you would be working with a group of patients over the phone providing teaching, coaching, and motivation to help them improve their chronic conditions.
Responsibilities Include:
Triaging patient calls
Conduct regular condition assessments
Patient-Centered Care Plans
Monthly med reconciliations
Patient Education
Contact with providers as needed
Qualifications:
CMA or RMA License
Knowledge of third-party payer requirements
Familiarity with electronic medical records
Strong aptitude for detail and ability to work independently
Strong organizational, problem solving, communication, and interpersonal skills
Work Location:
Fully Remote
This is a 1099 role.
Job Types: Full-time Contract, 30+ hours weekly.
