LVN/LPN Care Management (Remote) at CVS Health
About the position
The Transition of Care Coach at CVS Health is responsible for coordinating care for members experiencing significant health changes, ensuring seamless transitions between care settings, and facilitating post-discharge goal attainment. This role involves collaboration with members, providers, and community organizations to address health and social needs, ultimately enhancing member care and supporting the growth of Care Management Programs.
Responsibilities
ā¢ Complete outreach cadence calls and post-discharge questionnaires within compliance timelines.

ā¢ Utilize reporting to identify utilization and reduce Emergency Department visits and hospital readmissions.
,
ā¢ Follow members throughout the care continuum after inpatient hospital admissions.
,
ā¢ Facilitate Interdisciplinary Care Team Meetings to discuss service needs and support transitions.
,
ā¢ Ensure members understand their medications and have received them post-discharge.
,
ā¢ Coordinate post-discharge meal delivery and assist with securing durable medical equipment (DME).
,
ā¢ Evaluate health and social indicators to identify barriers to optimal member health.
,
ā¢ Provide clinical assistance to determine appropriate services and supports based on member health needs.
Requirements
ā¢ 2+ years of LPN or LVN nursing experience
,
ā¢ Active and unrestricted compact LPN or LVN licensure in state of residence
,
ā¢ High School diploma or equivalent (must be an LPN or LVN)
Nice-to-haves
ā¢ Bilingual (Spanish)

ā¢ Self-motivated and energetic
,
ā¢ Discharge planning experience
,
ā¢ Advanced proficiency in Microsoft Word, Excel, and Outlook
,
