About the position

The LTSS Service Care Manager plays a crucial role in developing, assessing, and coordinating holistic care management activities for long-term care members. This position focuses on enabling quality and cost-effective healthcare outcomes by creating personalized service care plans, educating members and their families about available services, and ensuring that care plans are monitored and adjusted as needed. The role requires local travel within a 30-mile radius and is primarily remote, with a preference for bilingual Spanish speakers.

Responsibilities
• Assist in developing, assessing, and coordinating holistic care management activities.

• Develop or assist with developing personalized service care plans for long-term care members.
,
• Educate members and their families/caregivers on services and benefits available to meet member needs.
,
• Evaluate the needs of the member and recommend a plan for the best outcome.
,
• Identify providers, specialists, and community resources needed for long-term care.
,
• Coordinate between the member and care provider team to ensure services are accessible.
,
• Provide resource support to members and their families for various needs.
,
• Monitor care plans and member status, providing recommendations based on identified needs.
,
• Interact with long-term care healthcare providers to ensure member needs are met.
,
• Collect, document, and maintain long-term care member information for compliance.
,
• Perform home and/or site visits to assess member needs and collaborate with healthcare providers.
,
• Facilitate education to long-term care members and their families on healthcare options and benefits.
,
• Provide feedback to leadership on opportunities to improve quality of care and service delivery.

Requirements
• Bachelor's degree required.
,
• 2 - 4 years of related experience or equivalent experience.
,
• Valid driver's license required.
,
• LPN or RN preferred.
