LTSS Concierge Coordinator - Kansas
About the position
The LTSS Concierge Coordinator plays a crucial role in supporting the LTSS Coordinator by facilitating the person-centered planning process for individuals enrolled in specialized programs. This position involves developing and monitoring service coordination tools, engaging with stakeholders, and ensuring that individuals receive the necessary support to meet their health care needs and personal goals.
Responsibilities
Ā Perform telephonic and/or virtual outreach to individuals in specialized programs, providers, or other key stakeholders.

Ā Submit utilization/authorization requests to utilization management with necessary documentation.
,
Ā Consult with the primary service coordinator to monitor the Person-Centered Support Plan (PCSP) for identified risks.
,
Ā Engage with clinical healthcare management and interdisciplinary teams for care coordination support.
,
Ā Manage non-clinical needs of members with chronic illnesses and disabilities.
,
Ā Document members' short- and long-term service and support goals in collaboration with their care team.
,
Ā Identify members who may benefit from alternative service levels or waiver programs.
,
Ā Serve as a mentor or subject matter expert for new staff and assist in training initiatives.
,
Ā Report critical incidents to appropriate internal and external parties.
,
Ā Assist in appeals, grievances, and state audits.
Requirements
Ā BA/BS degree and a minimum of 2 years of experience working with a social work agency.
,
Ā Any combination of education and experience providing an equivalent background.
Nice-to-haves
Ā BA/BS degree in a health care related field preferred.

Ā Specific education, years, and type of experience may be required based on state law and contract requirements.
