LPN/LVN (Case Management Analyst)
Job Title: LPN/LVN (Case Management Analyst)
Location: 100% Remote
Duration: 3 months (possible extension)
Pay Rate: $27/hr on W2
Schedule: Monday - Friday 8-5pm CST, will need to work every 6th weekend (both Saturday & Sunday - will have days off during the week)
Training Schedule hours will be the same
Job Description:
Ā Delivers professional activities in the Case Management job family.
Ā Coordinates enrollee care needs across the continuum for assigned members who are medically stable but require primary psychosocial support and acute intermittent medical support.
Ā Interviews members and relatives to obtain social history relevant to medical problems and care planning.
Ā Assists members with environmental difficulties that interfere with obtaining maximum benefits from medical care.
Ā Serves as a liaison between primary care team, members, relatives and appropriate outside agencies.
Ā Applies standard techniques and procedures to routine instructions that require professional knowledge in specialist areas.
Ā Provides standard professional advice and creates initial reports/analyses for review.
Ā May provide guidance, coaching, and direction to more junior members of the team in Case Management.
Qualifications:
Ā Current Non Restricted LPN or LVN License.
Ā Excellent computer skills
Ā Prior Authorization Experience
