Responsibilities:
As an LPN Home Health professional, you will be responsible for:
Verifying the plan of care with the RN prior to each visit
Collaborating with physicians and RNs to ensure patient needs are being met
Teaching patients and caregivers self-care techniques
Checking vital signs and reviewing medications
Performing specialized procedures ordered by physicians
Preparing equipment and materials for treatments using aseptic technique
Completing documentation in a timely and accurate manner
Qualifications:
To be considered for this role, you must have:
One year of clinical LPN experience
Current LPN license, specific to the state(s) you are assigned to work
Current CPR certification
Valid driver's license, reliable transportation, and liability insurance
