Responsibilities
Ā Verify plan of care with the RN prior to each visit.
Ā Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
Ā Teach patients and caregivers self-care techniques.
Ā Check vital signs.
Ā Review all medications and perform a medication reconciliation at each visit.
Ā Perform specialized procedures ordered by physician (i.e. foley catheter change).
Ā Prepare equipment and materials for treatments using the aseptic technique.
Ā Complete documentation timely and accurately.
Qualifications
Ā One year of clinical LPN experience.
Ā Current LPN license, specific to the state(s) you are assigned to work.
Ā Current CPR certification.
Ā Valid driver's license, reliable transportation and liability insurance.
