Licensed Practical Nurse (LPN) – Remote Chronic Care Manager
Position Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team as a Care Manager supporting CCM and BHI programs. This is a part-time contract role requiring approximately 40 hours per month, working with a patient panel of 35–40 individuals. Experience in care coordination or patient support is recommended but not required—reliability, strong communication, and a motivated mindset matter most.
This contract also offers potential for extension as we bring on additional providers, increasing caseloads and care coordination needs. If you enjoy this role, there's opportunity to grow with our team.
Key Responsibilities
• Perform outreach through phone or telehealth to schedule follow-ups and confirm care plan progress.
• Enter patient data and encounter notes into the EHR or secure spreadsheet systems.
• Assist patients with navigating care resources and escalating needs to clinical supervisors.
• Track time logs and complete documentation in line with Medicare billing standards.
• Coordinate with providers and care team members to maintain workflow compliance and operational efficiency.
Qualifications & Skills
• Valid LPN license
• High school diploma required;
• Effective communication skills, comfort with phone-based patient interactions.
• Familiarity with EHRs or spreadsheet documentation (preferred but can train).
• Compassionate, detail-oriented, dependable, and able to work independently.
• Prior care coordination or chronic care experience helpful but not mandatory.
Job Type: Part-time
Pay: $25.00 - $35.00 per hour
Expected hours: 10 per week
