What We’re Looking For
• Licensed Physical Therapist
• Degree from an accredited Physical Therapy program (DPT, MSPT, or BSPT)
• CPR certification (preferred)
• Experience in home care, Part A or B, or outpatient.
• Reliable transportation
• A compassionate, patient-centered mindset and strong communication skills
• Familiarity with electronic health record systems (e.g., WebPT or Hello Note)
• New Graduates are welcome to apply
Key Responsibilities:
• Evaluate and treat patients with physical disabilities, injuries, or functional limitations
• Develop and implement individualized physical therapy care plans
• Provide therapeutic exercises, manual therapy, gait training, and neuromuscular re-education
• Deliver orthopedic, post-surgical, and neurological rehabilitation services
• Educate patients and caregivers on home exercise programs and safe mobility practices
• Maintain documentation in electronic medical records in accordance with Medicare and CMS regulations
• Assist with fall prevention, balance training, and pain management strategies
