Licensed Behavioral Health Care Manager
To support a growing collaborative care team, the full-time Licensed Behavioral Health Care Manager will engage with cancer patients through telehealth, conduct biopsychosocial intakes, and coordinate care between specialists and internal teams in a remote setting.
Key responsibilities:
⢠Conduct initial clinical member biopsychosocial intakes and administer behavioral health assessments
⢠Lead weekly interdisciplinary team meetings and ensure high-risk members are discussed with the Consulting Psychiatrist
⢠Document member progress in a HIPAA-compliant electronic medical record system and facilitate referrals for additional services as needed
Required qualifications:
⢠Active, unrestricted independent clinical license in at least one U.S. state (e.g., LCSW, LPC, LMFT)
⢠Bilingual in English and Spanish
⢠Bachelor's degree in nursing, social work, or related health and human services discipline; Master's degree preferred
⢠At least two years of clinical care management experience, preferably with cancer or advanced illness patients
⢠Strong organizational skills and adaptability to a start-up environment
