Job Overview:
We are seeking a dedicated and empathetic Hospice Care Nurse to join our team at Amedisys, a leading home health and hospice company in the U.S. As a Hospice Care Nurse, you will have the opportunity to make a meaningful impact on the lives of patients and their families, providing high-quality care in the comfort of their own homes.

Responsibilities:
Perform patient assessments and develop individualized plans of care
Collaborate with the care team to provide comprehensive care and support
Promote patient health and independence through teaching and rehabilitative measures
Supervise LPNs and HHAs, and provide staff education and training
Maintain accurate and timely documentation, and communicate patient progress to the clinical manager and care team
Participate in on-call responsibilities and provide on-call services to patients and families as assigned
Requirements:
Current RN license, specific to the state(s) you are assigned to work
One (1+) year of clinical experience as a Registered Nurse (RN), or approval by VP Clinical
Current CPR certification and valid driver's license, reliable transportation, and liability insurance
