Job Overview

We are seeking a dedicated and compassionate In-Home Physical Therapist to join our dynamic healthcare team. In this vital role, you will deliver personalized physical therapy services directly in patients’ homes, empowering individuals to regain mobility, reduce pain, and improve their overall quality of life. Duties
• Conduct comprehensive assessments of patients’ physical condition, including mobility, strength, and functional limitations
• Develop and implement tailored care plans that align with patients’ goals and medical needs
• Administer physical therapy treatments per MD protocols
• Monitor patient progress regularly, adjusting care plans as necessary to optimize outcomes
• Educate patients and caregivers on injury prevention, exercise techniques, and home safety strategies
• Document all patient interactions accurately
• Collaborate with our case managers and physicians/nurses to ensure comprehensive patient care

Skills
• Strong knowledge of physiology, anatomy, and medical terminology relevant to Orthopedic practice
• Proven experience in home health care, outpatient clinics, hospitals, or inpatient settings with a focus on patient care excellence
• Familiarity with physiotherapy techniques such as trigger point therapy and orthopedics treatment methods
• Ability to interpret medical reports and collaborate effectively within multidisciplinary teams
• Excellent communication skills to educate patients and support their recovery journey
