In Home Healthcare LVN- Low Acuity. Day Shift

Conduct patient assessments including physical, mental, and emotional states to tailor individualized care plans.
Administer medications and treatments as prescribed by healthcare providers and monitor reactions.
Record patient progress and consult with a wider healthcare team to adapt care plans.
Teach patients and their families on proper home health care strategies and preventive measures.
Maintain a safe, clean, and healthy environment for your patients.
Coordinate with healthcare professionals to ensure consistency and accuracy of treatment.
Proficient in patient health assessment
Strong communication and interpersonal skills
Detail-oriented with proficient record-keeping abilities
Ability to handle medical emergencies
Skilled in administering medication and basic patient care
