Responsibilities
Ā Coordinates and oversees all aspects of patients' home care.
Ā Collaborates with patient care teams to develop and implement care plans.
Ā Educates families on patient condition, needs and care.
Ā Performs head to toe physical assessments.
Ā Checks vital signs and manages medications.
Ā Gives injections, IV therapy or tube feedings.
Ā Administers and maintains catheters.
Ā Completes documentation timely and accurately.
Ā Supervises LPNs and HHAs.
Ā Regularly communicates patient progress to the clinical manager, physician and care team.
Ā Other duties as assigned.
Qualifications
Ā One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
Ā Current RN license, specific to the state(s) you are assigned to work.
Ā Current CPR certification.
Ā Valid driver's license, reliable transportation and liability insurance.
Ā Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
