Pathways is hiring an Implementation Project Manager to support program management and implementation activities for a portfolio of work focused on connected care technology, digital health services, virtual care tools, training, communications, and adoption support for the Department of Veterans Affairs.
Job Purpose
We are seeking an experienced, organized, and mission-driven Implementation Project Manager to support the planning, execution, coordination, and evaluation of connected care technology implementation initiatives across complex healthcare environments.
In this role, you will help manage implementation projects that drive awareness, readiness, adoption, and sustainment of digital health and virtual care tools among healthcare staff, Veterans, family members, caregivers, facility and network stakeholders, and other end users.
The Implementation Project Manager will support the development and execution of implementation strategies, project plans, readiness assessments, training programs, campaign toolkits, stakeholder engagement activities, and reporting deliverables. This role requires strong project management skills, healthcare implementation experience, excellent communication abilities, and the capacity to coordinate across multiple teams, stakeholders, and workstreams.
You will work closely with government clients, subject matter experts, product teams, field stakeholders, trainers, communications professionals, and implementation specialists to ensure projects are delivered on time, aligned with VA priorities, and designed for sustainable adoption.
Duties & ResponsibilitiesManage implementation projects supporting digital health, connected care, virtual care, training, communications, and technology adoption initiatives.
Develop and maintain project plans, schedules, trackers, status reports, meeting materials, and implementation documentation.
Coordinate implementation activities across internal teams, clients, stakeholders, subject matter experts, and end users.
Support planning and execution of readiness assessments, implementation strategies, deployment activities, and adoption efforts.
Track project milestones, risks, issues, dependencies, deliverables, and performance metrics to ensure timely and high-quality execution.
Facilitate meetings, trainings, office hours, workshops, and stakeholder engagement sessions, as needed.
Support the development and refinement of implementation materials, training resources, toolkits, job aids, communications, and reporting products.
Monitor stakeholder feedback, adoption barriers, and implementation challenges; recommend process improvements and solutions.
Prepare and present project updates, findings, recommendations, and implementation outcomes to leadership and stakeholders.
Collaborate with cross-functional teams to ensure alignment with project goals, client priorities, and contract requirements.
Apply project management, change management, implementation science, and human-centered design principles to support sustainable adoption.
Ensure all assigned activities and deliverables are completed accurately, professionally, and on schedule.
RequirementsBachelor’s degree required in healthcare, public health, health administration, business, communications, project management, information systems, implementation science, or a related field.
10+ years of professional experience in project management, implementation, healthcare operations, digital health, training, change management, or related fields.
Demonstrated success managing complex projects, workstreams, timelines, deliverables, risks, and stakeholder expectations.
Background supporting programs in healthcare, government, technology, consulting, or other complex organizational environments.
Strong ability to coordinate cross-functional teams and drive work forward in fast-paced, client-facing settings.
Skilled in developing project plans, status reports, briefings, trackers, implementation materials, and executive-level communications.
Proven ability to support adoption, training, deployment, process improvement, and change management activities.
Comfortable facilitating meetings, presenting updates, documenting decisions, and following up on action items.
Track record of building productive relationships with clients, partners, leadership, subject matter experts, and end users.
Proficiency with Microsoft Office Suite and collaboration tools such as Microsoft Teams and SharePoint.
Highly organized, detail-oriented, proactive, and able to manage multiple priorities with minimal supervision.

Preferences Master’s degree preferred in health informatics, public health, healthcare administration, business administration, project management, implementation science, or a related field.
Experience working with the Department of Veterans Affairs or other federal healthcare organizations.
Familiarity with VA systems, processes, stakeholders, or healthcare delivery environments.
Experience with telehealth, virtual care, connected care, My HealtheVet, digital health tools, or patient engagement technologies.
Project management certification, such as PMP, CAPM, CSM, or related credential.

Work Environment and Physical Requirements:

Prolonged periods sitting at a desk and working on a computer. Must be able to lift up to 15 pounds.

Travel: depending on client requirements, expect up to 15% travel time for the position.

Compensation:

Pathways offers a comprehensive benefits package including but not limited to health, medical, dental, vision, and 401k.

The compensation range for this position is $75,000.00 - $150,000.00. Exceptions based on experience and education.

Pathways 2 Solutions is a fully remote government contracting organization. Employees may work remotely from approved states where the company is authorized to employ personnel and maintain required payroll, tax, regulatory, and contractual compliance.
